- Enterprise business certification proof (if applicable)
- Professional license proof (if applicable)
- Minimum of 3 project references
- Current most fiscal year-end financial statement
- Audited or CPA reviewed statements preferred (consist of a balance sheet + income statement)*
- Current year W9
- Letter from your bank stating your relationship
- EMR Verification
- Letter from vendors insurance agency stating their EMR score for the last 3 years *
- 3-year loss run report from vendors insurance agency if EMR is 1.0 or less, TRIR of less than 3.9 & LWDCR of less than 0.7
- OSHA 300A logs from the last 3 years
- Current safety manual
- Current substance abuse policy
- Job hazard analysis (if applicable)
- Current year COI*
- Generic bonding letter from vendors surety company stating their current limits, dated within the last 6 months (if applicable)*
- Current HUB participation plan (if applicable)
- New Vendor Request Form
- Master Construction Subcontract
- Subcontractor Safety Requirements
- *Must be updated during renewal period yearly
